The county of Lincolnshire, UK is improving the well being of its citizens by facilitating collaboration among health and social care organisations to transform services. Lincolnshire Health and Care (LHAC) believes that connecting patient data and making it accessible online to a variety of stakeholders enables the highest quality healthcare for all residents. To achieve this, LHAC is modernising access to data for 700,000 caregivers and, ultimately, their patients across the East of England through the introduction of the Care Portal, the community’s solution for interoperable healthcare data.
In 2016, LHAC selected InterSystems HealthShare to power the Care Portal initiative, with J2 serving as InterSystems’ implementation partner for the project. In close collaboration with InterSystems, J2 designed and built a platform for LHAC that provides patient data to hospital and clinical staff via a full-featured web interface. Behind the scenes, the Care Portal combines data from all providers within the county, creating a single, complete record that is easy to read and understand. This single record eliminates the need for redundant tests and x-rays, saving a projected £23 million over five years. J2’s responsibilities included establishing LHAC’s HealthShare systems infrastructure, developing the necessary interfaces to integrate participant data, and customising the Care Portal user interface to meet the LHAC community’s specific requirements.
More information on the Care Portal, including an introductory video, is available on the LHAC web site.
