SCL Health, founded in 1972 and headquartered in Broomfield, Colorado, is a $2.5 billion nonprofit healthcare organization founded with a commitment to serving the poor and vulnerable. It operates eleven hospitals, 210+ physician clinics, home health care, hospice, mental health care, and safety-net services in Colorado and Montana.

J2’s work at SCL Health began with a best-practices consulting engagement that covered a number of critical topics related to SCL Health’s use of InterSystems’ Ensemble (now Health Connect) technology. Our analysis and recommendations included server sizing and configuration, high availability, and disaster recovery; Ensemble configuration and performance tuning; and strategies for distributed interface development across a geographically-dispersed team. In addition to thorough documentation of our findings and document templates for managing integration projects, we provided SCL Health with our Subversion-based source code control solution, which integrates Subversion directly within the Ensemble development environment and supports structured code promotion from development sandboxes to test, staging, and production environments.
After partnering with J2 to establish an interoperable health information exchange on InterSystems HealthShare, SCL Health asked J2 to help drive innovation in population health using Salesforce to improve data sharing and care coordination across the health system. J2 built two Salesforce solutions for SCL Health that drive measurable improvements in hospital readmission rates, medication adherence, and patient satisfaction scores: Meds-to-Beds and Care Share.
SCL Health’s Meds to Beds program identifies patients at high risk for readmission due to a lack of adherence to medications and other treatment follow-ups. Working closely with the health system, J2 designed a workflow application that shares data from the hospital EMR and outpatient pharmacy platform with Salesforce Service Cloud. Salesforce alerts the pharmacy when patients are leaving the hospital so that their first outpatient refills can be delivered bedside.
Care Share tracks patient activity and provider workflow to assist care coordinators as they provide guidance and instruction to joint replacement surgery patients at every stage in their recovery. Using Salesforce Service Cloud, J2 built application workflows that help SCL Health stay connected with patients throughout the rehabilitation process. Because the application was designed to support a broad range of procedures and chronic illnesses, SCL Health plans to adapt Care Share to other critical care coordination workflows and care management processes across multiple facilities and departments.
By partnering with J2, SCL Health harnessed the power of the cloud to improve care coordination, increase patient engagement, and deliver better health outcomes across a diverse patient population.
